As I mentioned in version one of the Problem Statement, there are important gaps which place a burden on the job seeker who is, arguably, least equipped to address the system’s inadequacies. I revealed the principal communication problem in a flow-chart diagram on said version 1 page as the four failing opportunities for feedback. Fifty percent of the time you won’t receive good feedback, and in real world cases this may go all the way: no feedback, none, zip.

Why is feedback necessary?

Feedback is a fundamental component of communication, and there are numerous cases of complex problems where problem owners go to extraordinary lengths to get it. These include bringing a new product to market, or running for public office. These situations involve high stakes, because they require monetary investments, involve many actors, and develop over long periods of time. For example, there is no certainty if a marketing campaign for a film will interest movie goers, or if a politician’s public policy platform will pass through the noise of rivals and meet the concerns of the voters. Nevertheless, problem owners in these contexts are notorious for their efforts to collect feedback. Film companies use pre-screenings to test their films before general release. Politicians use public polling in a discipline called political forecasting. Employer’s are keen to give feedback to their current employees. Employer’s want feedback for their commercial efforts. Why don’t employers develop relationships with prospective employees?

Below is another attempt at the Actor Table method–modified slightly–with two more columns: ‘wish’ and ‘crisis’, plus the ‘gap’ between them, and the ‘dilemma’. As I stated earlier, I added these additional columns, because they helped me to conceive of a narrative for each stakeholder. There are multiple entries for the same actors.

[3]: I used my experience trying to get a referral for myself from my medical medical doctor to imagine this customer. My doctor is employed at a medium size cluster of medical offices all under one parent group. Some 50-100 or so employees and a number of specialists in just this building. I needed to see a medical specialist for severe heartburn. When I called on 09-18-2014 for a referral from my primary care physician, I was told the process took 24-48 hours; It took more than a week. By Wednesday 09-24-2014 when I still hadn’t heard back, I traveled to the doctor’s office (1.5 hours one way on public transportation), because I wasn’t getting anywhere on the phone.

While trying to resolve the issues I was told by a receptionist the issue was out of their hands, and in the system waiting for the doctor to reply (if you know the office this also includes the doctor’s assigned nursing staff who have previously called me personally on behalf of my doctor). Is this a Complex Problem? You decide. The doctor’s actual desk is less than 200 steps away. It seems employee’s responsibilities (established by the company? or the employee? or the union?) do not permit them to step out of their desk role to investigate–even after 4.5 working days.

This would only be ludicrous if it wasn’t for Thomas Eric Duncan, the first Ebola patient to die in the U.S. I finally saw the specialist on 09-29. Duncan’s ordeal started on 09-25 when he was sent home and not until 09-30 was he properly diagnosed. Here’s a timeline from the Duncan case. That Dallas hospital has a serious problem now.

Maybe Duncan may have lived if the information collected by the nurse about Duncan’s trip to Liberia, Africa was provided to the attending doctor. Instead, the doctor is reported to have asked the same questions and received different answers (he told the nurse he just traveled from Africa, he told the doctor he lived in Dallas; both were true). I experienced this gap in patient care personally when I was told by a health care worker, Reception is one group and Doctors, just 200 paces away, are another group–the computer system handles the rest.

What’s more, a computer system inadequacy is also at the start of the story. I called on the morning of the second day (09-19). Later in the afternoon I called to find out if anyone had tried to contact me (I have terrible cell phone service and an even more terrible phone). The receptionist said flatly the scheduling system did not ‘record calls’ to patients. I pressed for clarification, You mean it hasn’t recorded a call yet? Or your system doesn’t record calls to patients? The answer I was given, I can’t tell if someone called you. Then the receptionist said, Check caller ID.
